Passport application requires a prescribed set of documents which have to be submitted along with the duly filled passport application form. In order to apply for a passport, a person can initiate a request by visiting the nearest Passport Seva Kendra

There seems to be a confusion on the documents accepted as valid proof of address and proof of date of birth (DoB) by the Passport Seva Kendra, Ministry of External Affairs, Government of India. Some individuals mistakenly carry those documents as a proof of address and date of birth which are not admissible by Passport Seva Kendra or the respective registered Passport Office.
Passport Seva online has a specified list of documents which are admissible as valid proof of date of birth. Which implies, a person can carry/attach any of those documents to fulfil the criteria of date of birth proof.
The birth certificate issued by the Registrar of Births and Deaths or the Municipal Corporation or any other prescribed authority, whosoever has been empowered under the Registration of Birth and Deaths Act, 1969 to register the birth of a child born in India are some of the most commonly admissible documents which can be submitted as a valid proof for verifying the date of birth of an individual.
Other than this, a person can submit transfer/school leaving/matriculation certificate issued by the school last attended/recognised educational board, Aadhaar card/e-Aadhaar card, election photo identity card (EPIC) issued by the Election Commission of India, PAN card issued by the Income Tax Department and Driving License issued by the Transport Department of the concerned state government.
Individuals who are not having any of the aforementioned documents willing to apply for a passport can do so by providing other authorised documents such as policy bond issued by the Public Life Insurance Corporations/Companies having the date of birth of the holder of the insurance policy and a declaration given by the Head of the Orphanage/Child Care Home on their official letterhead of the organization confirming the date of birth of the applicant.
Passport Seva Kendra also allows verifying date of birth by submitting a copy of an extract of the service record of the applicant (only in respect of government servants) or the pay pension order (in respect of retired government servants). Such copies of the service record should be duly attested/certified by the officer/in-charge of the Administration of the concerned Ministry/Department of the applicant.
